At the risk of sounding like a moron regarding subways

How do you effectively model them? Do you add them to an "above ground" layout? I love some of the sets I have seen but other than running them above ground, which makes no sense to me, how would one use them? Yes i am fully aware that plenty of lines run above ground so please don't bother pointing that out. Pictures would be appreciated as well.
