Is there a way to add Railsounds to a 1953 Lionel NW2 switcher? Anyone make a kit to do this, or would it be easier to find a Railsounds box car on ebay? I would like to have everything contained in one unit.

Shouldn't be a big issue, you can use the kit from Electric RailRoad to add the sound. Look at the RailSounds Commander kits.
They don't have an NW2 listed. I'm working on a similar project and it looks like the F3 is the closest sound kit they offer.
I like the C-420 sound cloud.
I see no reason why sounds could not be added to old P/W equipment. The only issue might be that the old solenoid based eUnits might place an inductive voltage spike on the power supply that could zap the sound board. You may need some filtering and TVS protection to protect the board against damage from these transients.

You might be better off tracking down one of the Lionel PWC NW2 switchers. They have Rail Sounds and TMCC. I have NYC version. The PWC NW2 has nylon gears on the power truck. This has always worried me.
I would think the old E Unit would have to go and an electronic E unit used if you installed an Electric Railroad Kit. Space under the hood is at a premium when it comes to the Lionel PW NW2 switchers.

I am considering installing an Electric Railroad kit in my Lionel Milk car. Question - When operating the milk car with your kit do you have to reduce the track voltage in order to prevent shooting milk cans across the train room?

You can set the voltage and duration of the coil pulse to tune the ejection of the cans.
