pins #4-2(-) and #4-4(+) go to the fan if I remember right. anyway it doesn't matter as maybe try running the fan from a separate source unplugged from the board and see if it works first.
I've only done 1 PS2 upgrade, but I'd check page 25 and make sure you didn't switch the red, green and blue connectors. Also the info on page 27 about cutting traces and checking page 47? I'm sure you did these things, but that's what I would be checking.
BRice, you should look up a member that goes by GGG. He does PS2 coversions.
Brad, sent you an e-mail. The fan get's +5VDC, the heater element and light have a higher voltage (18 to 22VDC) that is available, but it is limited to an effective 6V for lighting via the return to ground via the FETs and line driver chip. G

I assume fan just is not running?? or is it in the wrong direction?

GGG gave some invaluable help. The problem was narrowed to a bad smoke motor.
