On a smaller layout where you want to minimize track grades between levels, 5" railhead to railhead vertical separation may be enough, but check it with your own trains and track and roadbed thickness. Larger scale-size trains may need more.

 

I think my tallest car is a Lionel 9602 hi-cube boxcar, which stands 3.75" high from the railhead.

 

More separation might look better depending on the overall scenic treatment of your layout and the effect that you want.
