Theoretically, I know what to do. Practically, I am totally lost.
I am asking that those who have done this many times to share their knowledge with photos.

You're asking a question that's much too broad!
What specific locomotive are you dealing with?
I can give a broad answer, having done dozens.
Most difficult part is pulling the old axle gear, pressing on the new one, and accurately re-quartering. You need a quartering jig. I suspect you can search OGR back issues; I am sure I did an article or two in the early 1990s.
Easiest part is installing the motor. I use aircraft safety wire and a piece of old inner tube. I will look in to posting a photo or two when I wake up.
Changing the motor can be very easy. Changing the gearbox is very hard without the right tools and real know-how. I'll let Bob2 weigh in on that!
