Who can fix it, estimate cost, and do you need the entire engine or just the tender?

There is a guy on this forum that is an expert MTH repair technician. I believe his name is John and he works for Hennings Trains. If you look through a few posts you should locate him.

+1 GGG has fixed many of my MTH engines at a very reasonable cost.

I found rayman in gardner ks... I can drive to him and hand deliver so avoid shipping. I saw he had good cred...and meeting him personally he is a super guy and has some very cool G scale stuff of his own.

Can't argue with not shipping stuff, while most stuff gets here OK, I've seen a couple of bad examples. If you do ship, proper packing is KEY! Frequently, you have to do some work inside the manufacturer's box to secure the locomotive BEFORE putting it into the shipping box. If anything moves when you shake it in all axis, you're not done packing!
