The first iteration of The polar Express locomotive had two traction tires. There is one on the front right driver and one on the back left driver.
My Series 1 PE Berkshire has traction tires on the rear two drivers. All diagrams on the Lionel Support website show Series II through Series V with the traction tires on the rear two drivers... Series 1 is not broken down or illustrated, but mine has the original small Series 1 motor.
Rob, I got the polar Express when it first came out and have never had to change the traction tires. We only run it on Christmas time. One of us has a rare factory error!
Same here, & I have never changed a tire either, and it has a lot of track time... tonight included.
I'd like to see photos of your tire placement.
How about the other side?
At the top right of my photo is one tire and the second is at the bottom left . All other drivers are unfettered.

I doubt it was an error, it would be very difficult to do this by mistake. Missing one tire wheel would be plausible, but to find another one in the wrong spot on one loco would actually require 2 errors. The front & rear axle assemblies are different, the rear has the worm wheel.
Having the tires arranged like the one Mitch has will reduce tire wear as there is so much slop between the front and back axles that the differential action in turns isn't contributing to the wear.
