I bought a DZ 1090 signal bridge for my layout - it looks amazing, but I can't get it to show green/proceed.

To test the unit, I connected the red and black wires to hot and common ports of a CW transformer - when I did so, the semaphores both moved to the 'red' stop position and stayed there. When I turned power off and on again, the semaphores rose to green, then returned to red again.

I tried the reset instructions (attaching all 5 wires to hot and turning power on) and this did not fix the issue (note that the semaphore did not go to yellow when I did this either).

Has anyone else had this issue or has any idea what I should try next?

What kind of lighting do you have in your basement?   I had the same issue, just could never get the bridge to work with my florescent lighting.  Finally I had Dennis modify the programming to ignore the built in sensors and only accept from the two wires (White/Yellow) either by an isolated outside rail or Zstuffs DZ-1070

You can see my setup in this video.  Bridge input wires connected to DZ-1070, one for each track.
