Has anyone ever made use of a Failure Mode and Effects Analysis (FMEA) tool to document what happens when a part fails on an engine? This is a key quality tool that breaks down failures, detailing their effects on operation, along with a ranking of severity, likelihood of occurrence and detectability of occurrence before it happens. The software is normally MS Excel.

It seems the forum is mainly about failures and I thought this might be useful in summarizing many of the questions and answers. To use you will need to be very knowledgeable about the DCS system and how it works. Unfortunately, that is not me. I am happy anytime the system works correctly. Beyond that I usually need assistance.
