So I had a problem with switches not responding to controllers. I added the direct power source and all worked fine. Suddenly one switch locked in the straight position (I cannot even turn it manually when power is off). Any ideas on what may have happened?

Too add to what Larry said, the motor is likely jammed at the end of the toothed " segment gear" mechanism.
I had this issue and the switch motor cover was not completely snugged down, there are 2 screws that hold it in place. They were just slightly loose and the motor could rock up and turn past the mechanism and get jammed.

I've also added a tiny bit of grease to the motor pinion gear and pie teeth to help smooth out the switch action.
If the above tips don't work, it may be that the limit switches are allowing the pie plate to move past the end of the teeth and jamming.

Yes, this. I had a brand new Fastrack O36 remote switch jam the motor pinion teeth against the segment gear because the motor cover was slightly out-of-shape. I substituted a different motor cover which holds the motor correctly in place and all is good.
