Use a heavy duty side cutter and knurl the end of the control shaft with some 45 degree gashes in the metal. There are two types of retaining sleeves, a straight collar type, and a later thicker stepped collar. If it is the first type, they don't hold the shaft very well.
Just saw this posting, Steve, and suggest you follow Chuck's instincts. If the sleeve's inner diameter is worn why not replace it? It is cheap and easy to install. I use a drill bit extender over the stem of the switch shaft and tap the sleeve into place.
