As I recall, that horrible tragedy occured due to several factors that contributed to a "perfect storm": an overworked and inexperienced motorman, a sharp curve and lack of overspeed automated braking at that point on the line. The name of that stop was later changed from "malbone", which ironically translates from the French for "painful bone"! The article in the link above also mentioned a strike which resulted in an unusual consist makeup that contributed to the large number of fatalities.
