Did you set LABORED SMOKE to AUTO in the CONTROL MENU? From page 37 of he DCS O Gauge Companion 2nd Edition:

LABORED SMOKE:
• The EDIT LABORED SMOKE screen is displayed
• Pressing soft key S1 (OFF) turns Labored Smoke off for the active PS2 engine
• Pressing soft key S2 (AUTO) sets Labored Smoke to automatic operation for the active PS2 engine. The active PS2 engine will produce greater smoke volume when the Labor Rev Up key is pressed and reduced smoke volume when the Drift Rev Dn key is pressed
• Press the thumbwheel to lock in the labored smoke setting and return to the CONTROL menu

LABORED SMOKE is set to AUTO in the CONTROL MENU. It only works in MANUAL when the Labor Rev Up/Drift Rev Down rocker button is pressed, but doing this disables the Automatic Proto Chuff as stated on p. 41 of the manual. The problem is that when Proto Chuff is set to AUTO it doesn't result in any change in chuff sound or smoke output when the thumbwheel is rotated. From the description of Proto Chuff on p. 41 of the manual, rotating the thumbwheel to increase or decrease engine speed is supposed to result in an incease or decrease in chuff intensity and smoke output, respectively. Page 132 in the manual says to set Proto Chuff to Automatic. OK, I do that. It ain't working. Only works when the Labor/Drift rocker is pressed.

And, no, I haven't tried a Feature Reset because the manual says nothing about that.

Page 41 also says of Proto Chuff that:

Use of the one-touch LABOR REV UP/DRIFT REV DN rocker switch on the handheld overrides this menu setting. You will need to re-set it to Auto after exercising manual control.
