Canadian Transportation Safety Board has issued two urgent Safety Advisory letters to Transport Canada regarding the accident at Lac-Megantic. They revolve around procedural issues that are in compliance with the current rule/regulations but could still contribute to disasters like the at Lac-Megantic. aka you could be in compliance with the rules and still wind up with a colossal mess.

There is additional information in terms of details that I have not seen published previously but it still leaves a lot of unanswered questions like how the train broke apart (coupler failure?). Leaving a heavy train "parked" on a grade unattended and not secured seems to be pretty stupid.
