If you troubleshoot at the component level, member fec fan posted this schematic years ago for MTH 3-aspect signals with the timed-yellow feature:
For the signal heads which double-up on each LED like your 30-11005, the same circuit applies except each LED in the diagram is a pair of LEDs. I'd think the 2 LEDs of the same color would be wired in series as this would save power compared to the 2 LEDs in parallel.
I'm with John H on this one... I'd look for a broken wire connection first.
Presumably everything else works as intended...it changes from all dark to Red when triggered, then from Red to Yellow when untriggered, then waits 10 seconds (or whatever the delay), then Yellow goes out ... but Green does not come on. If all this occurs and wiring appears intact, then I suppose one of the Green LEDs might have failed which would take out both (if wired in series as I suspect). In that case Green LEDs are a few pennies ... you will pay much more for minimum order quantities and shipping.
If you do get a chance to open the signal head itself where the LEDs are, I'd be interested in a photo of the LED wiring. I don't know if/how the head itself comes apart to access the LEDs.
