Problems with Smoke Unit on Lionel Hot Box Reefer

I own a Lionel Hot Box Reefer, which I didn't use for many years. When I tried running it the smoke unit wasn't running, but the lights and sounds worked fine. I took it apart to discover that I could get the smoke unit to work in neutral, but it required 12 volts or more of power. It seemed to work really well if I turned the power all the way up (20 volts). When running the car I don't want to give it that much power because my train will run too fast. Even at 12 volts running the fan has difficulty staying on. I don't remember this happening many years ago when I first bought the car. Does anyone know why this happening?
