When GE rebuilt the U23Bs into Super 7s, did they keep the original carbody, and graft on a new cab and radiator section? Or was an entirely new shell fabricated? In other words, if I were to kitbash this engine, would I start with a U23B donor locomotive (e.g. from Atlas O) and just add the cab and radiator from a narrow-cab DASH-8, or would I need to craft an entirely new body?

As far as I know, the carbody is all new from GE.  They used the frame, trucks, etc from the U-boats but the cab and hood are the more squared-off Dash 8 style.  

To answer your second question, the hood and radiator section have differences from the B40-8, so if you want an accurate model you'll probably have to scratch most of it.  The cab might even be the only "stock" part, I'm not sure if the short hood is the same between the two.
