Lionel USRA 2-8-2 4501, sku 2131370. Has had no problems with any track until 3 days ago, and now it consistently derails on this 0-36 RH switch track. I've tested and tested, and it just appears that right wheel gets behind the switch and pulls it out. Included video and pictures, can anyone see an obvious problem?

Just replaced traction tires...could they be too thick thus lifting the truck wheels?

Spring on truck looks intact and functional. Truck joints oiled and move freely.

Video: Sorry, start at 0:17. Truck wheels are clearly not on track properly in beginning of video.20211217_19423420211217_19424320211217_19430220211217_19431820211217_19434620211217_19441620211217_194434

I've found this to be a common issue with certain locomotives and many FasTrack Switches.  I've modified about 30 of them to work without issues even adjacent to curves.  Since yours appears to already already be embedded in scenery, my first suggestion would be to check the wheel gauge of the trailing truck to make sure it's within spec.

My 2 cents.........Seems to me to be more of the switch being an issue.  Coming off of a curve going to a switch can be problamatic occasionally.

Looking at the switch without the engine it looks like the inside switching rail needs to be slightly bent closer to the inside rail or filed slightly to be pointed more evenly with the inside rail..........or........... the switch might have some debris or obstruction that is now keeping it from fully closing. ( from the top photo of the switch ,it seems to be a pretty good scrape mark on the tie from the inside switch rail )

Good luck with this .  Keep us updated.

Slightly bending and filing points' tips are part of what I've done to eliminate the gaps between the rails and points.  Some times there is also excess plastic flashing inside the switchstand roadbed extension that limits the travel of the sliding grey plastic rack gear that rotates the flag/lantern.

Here are some pics of modified points and a switch:

Since these modifications require access to the bottom of the switch, that's why I recommend starting with the wheel gauge first.
