What is an effective way to create a transition track from MTH RealTrax to Lionel FasTrack?  

Here is a suggestion from Bob Young that I found in a now closed thread:

"Lionel makes a transition piece that allows connection to other track.  One end fits fastrack and the other fits old tubular track, while the other manufacturers make a transaction or adapter track where one end fits their track and the other ends fits old tubular.  So a piece from each manufacturer fit back to back will make the connection between the two..."

Is that still one of the best solution?

Finally, is there a commercially available transition piece that I haven't been able to locate?

For a layout, a butt connection using a section of each dressed flat is probably the neatest and fastest.  Using a half-section of each cut cleanly through the center will give you two such transition sections, and they can optionally be fixed/cemented together and then dress the rails at the transition.
