Years ago OGR had an article about making a car creeper into an elevated creeper so you could roll around under your bench work. I laughed at it because I was young and pliable. Now that I'm older and more decrepit,  it seems to be a great idea, plus a good place to nap.

Trying to find the plans, anyone remember which issue?

Why not simply purchase a good quality automotive Creeper at a local auto parts store.

Buy a creeper with good cushioning and head support. Figure out the height you want to raise it up so you are comfortable, still have clearance, but not a long reach. Build a box with 2x4's and plywood at least the length of the creeper with cross bracing anywhere between 16 to 24 inches spacing to support your weight and rigidity of the box.  I would make it probably 6-12 inches wider than the creeper so it doesn't want to tip over with the added height. Mount the casters on the bottom of the box and mount the creeper centered on top of the box.
