Probably not news for you real railroad workers (as opposed to us armchair foamers) who have to be super aware of your surroundings as one slip and tragedy.

There are many terrible ways to die on the railroad, and being coupled up between two cars is one of the worst I can think of. Being rolled between two cars at a turnout is another.
There are Rules and Safety Rules intended to prevent most or all of the ways, and there are specific ones that can keep you from being coupled up or rolled if you observe them.
I sympathize with this victim and his survivors, but somewhere, somehow, he didn't think of the rules.
You can die from someone else's rule violation, or you can die from an act of God, and the Rules can't do anything for you in these instances. The best you can do is know the Rules, observe and obey them, and hope for the best.
