Please,,,,,,,,,,,lets just wait for the NTSP investigation to be completed, and the appropriate report to be issued. There has already been WAY TOO MUCH SPECULATION about this disaster.

Like most accidents, an unfortunate combination of events cause it. Why would you post a title that sounds like an assumption?

There's a whole pile of human error in this incident.
