It's happened more than once on my layout(LOL)
Could be a simple explanation the train was going so slowly that the switchman thought both of the wheelsets are over so he switched it before the other wheelset was through, it was going so slowly, it stopped!other wise the train would be twisted and more damage would have occurred.
Truly remarkable that this wasn't a very serious accident. IMHO
