both work fine with an mth z750 horn and bell on the controller

Are you using a powermaster? Can you control the track voltage with it?
Yes. Absolutely. Runs fine.
It's possible that the whistle and bell buttons on the CAB-1 have failed. The most used buttons often do that.
Can you get your hands on another Cab1 to try out?
This happened to me with my Cab1. It is the carbon pads under the whistle that needs to be replaced. Easy to do here is a video, I got my replacement pads off eBay.
Take a look at KeyPad-Fix, worked for me.
Not worn out buttons. I moved the engine to another track and moved the same power setup to that track. Horn sounded with no issue. What could be the issue with the other track? Makes no sense
Track polarity reversed?
I Tried that. Its crazy
