As far as I know direct replacements are not available.
However there is a unit that can be made to fit.

You could consider the retrofit of a fan driven smoke unit, lots nicer smoke and the possibility of synchronized chuffing as well.
There is barely room for the Seuthe smoke unit.
Thanks for the info Carl. I haven't the machines to perform that work, so I guess my customer is SOL. But your modification looks great. Glad you were able to make it work.
Don't give up so easily. MTH makes an HO smoke unit that can be made to work. I don't think you will have a problem installing it if they can fit it inside an HO Mikado. Lionel really needs to do something about their electronics. I'm pretty sure Jon Z. is working on it. There really isn't any reason they need to be so large. Even that motor is way oversized. You can order this smoke unit directly from MTH with the part number shown.

I was thinking of the MTH HO smoke unit, when I was down at the ASC Class I saw them, they were pretty tiny.
OTOH, there isn't much room in there, it would be a pretty tight fit!

show me what your looks like...just took one out of my 3rd rail prr q2..it works..id put in my mod mth smoke unit in it..if it can work in your train you can have it for free..
I am looking at the MTH smoke unit pictured by jonnyspeed. I went to the MTH web sight but could not find how to get to the parts list. When I put in the part number in the search it was not found.

I just checked on the technician parts ordering site, the unit is $45 list price and it's listed as available. You can call Midge and ask for the part.

You have to register for the parts site. Once you do you can order parts online. Or you can call Midge.
Thanks everyone for your suggestions, but there is virtually room only for the barrel of a Suethe unit and nothing more. If you look at the photo Carl posted the space between the front of the flywheel and the back of the slide switch just ahead of it is all the room for a smoke unit. Practically the size of the HO unit's motor and that would be about all. As it turns out I got this customer's working without needing to find a replacement unit.
