The two U terminals are common. The hot line for accessories is the B terminal. You're using the SC-2 as a simple switch here, so you'll wire it just like you'd wire an in-line switch.

In his picture, he obviously has this programmed for all accessories. If you have four switches and two accessories, the right hand two terminal blocks are Acc #1 at the top and Acc #2 at the bottom. The switches would use the four left hand connection blocks.
Well, it would depend on the current draw of the accessory. For most Lionel accessories that don't have an excessively long wire run, #22 would probably work fine, for longer runs or higher current stuff, #18 would be a better choice.
Any spade lug that fits the terminal strip. The current for virtually all accessories won't tax any spade lug that I've seen.

Sometimes getting larger wire into the Fahnestock clips is a problem, you have to try it. In any case, #22 wire should be plenty for that accessory.
