What would cause a TMCC steam engine with cruise to all of the sudden take off at top speed? Bad ground?

Powered up with no TMCC signal.

You may want to check the control wire connections at the base and track. If either or both of them are loose/faulty you'll lose TMCC signal as stated by Rob above. It's also possible a bad connection between the control board and ground in the loco is causing intermittent TMCC signal loss.
Also try another TMCC engine to see if the problem is the track/TMCC signal.
I would check the antenna connection and placement of the wire inside the loco too.

Top suspect if it's a stock TMCC engine would be the Odyssey I magnet ring just separated and now you don't have any speed control.
Try this. Power up without giving it any throttle. If it doesn't run away and recognizes the TMCC signal, move the throttle. If it then takes off, it's about 90% possibility that the Odyssey magnet ring just croaked.

Not stock. It was given the large Pit. motor from Harmonyards. Love it, but it took off on me na couple of times yesterday.

Cruise commander, Super chuffer...that's it, I think. It's been a few years. Kept the Railsounds board.
In that case, reprogram it and make sure you program the engine type after the TMCC ID, AUX1/4 for steam.
