If I send my new Lionel Berkshire back to Lionel for repair of the smoke unit that no longer works, will they just replace just the fan motor (3 blinking lights in succession 3 times and then no smoke), or will they replace the pad and clean the smoke unit as well? Do you have to return the tender as well?
Does Lionel recognize that there is a problem with the smoke unit, and if not, what can one do to prevent this from happening again?

Typically they will do any repair required. Including but not limited to check all the smoke unit components and cleaning and repairing where needed.
You would have to contact Lionel as to what they require for the repair.
I did two Berks tonight. One was my own. Lionel does what ever is needed to make your locomotive right. The tender is not needed unless they ask for it. If you have the 3 blinking lights, your smoke wicking will be undamaged.
