Uncoupling Box Car

I am looking for some help on a Lionel O gauge PS-1 standard box car that keeps randomly is uncoupling coming out of #5 Atlas switch. Any guidance or ideas would be appreciated.

Rob,

If this boxcar has a magnetic plunger, is it coming into contact with the switch anywhere?

Just an idea...

Thank you for the reply. The plunger is not making contact, but I did notice the wheel was loose on the axle, and it was riding up on the rail. I fixed the wheel and it seem to stop derailing.
