If I already have a 3D item is it possible to scan that somehow and make a 1/2 size duplicate?

There are 3D scanners, but they're not cheap. The easiest way is to have the file and just print it at 50%. Alternatively, if you got it from shapeways or something, then try messaging the seller and ask if they would re-scale it. A lot of times they will do this with no issue.

I have been watching the 3-D scanning apps that run on current iPhones with lidar sensors. Not really practical yet, but getting there.

If the object isn't to complicated, you could CAD it up and then scale to your hearts content.
