It would seem to refer to a Lionel engine that has been modified for 2-rail operation (insulated wheels, other changes).
The Lionel Smithsonian Dreyfuss Hudson and cars were offered in 2 rail

I forgot about those -- not really so familiar with Lionel. Doesn't the ad or offering say what engine it is?
The Smithsonian collection consisted of the Dreyfuss Hudson, the German 2-10-0 BR50, and PRR K4s #3768 with "torpedo" streamlining (6-18208). They were brass, and offered in 2-rail only.
There was a 3 rail Smithsonian Hudson, but both the K4s and BR50 were 2-rail only.

Just our of curiosity, I read years ago about converting Lionel to 2 rail and it involved insulating one side of each cars wheels of every loco or car you had, or not and running an outside third rail (which seemed counterproductive to me). In today's desire to convert to 2 rail, are there suppliers where you can get wheels already insulated on one side? How is it done today? I always thought that Marklin with their little pin in middle of each tie was a neat idea, but never really looked at how their turnouts and crossovers worked with clearing the shoe that contacts that pin
I wish that Lionel would do 2 rail like MTH.
Lionel JLC Challenger 28090 from 2000 was advertised in 2 rail, not sure if the 2 rail version was ever made.

Each manufacturer does their own. MTH has a split axle system I believe? I've seen some steamers where the tires are insulated on one side. The rest have a plastic insulator on at least one side of the (tender) wheels.
MTH, Atlas, etc., has two rail wheels. NWSL had them.
You can buy RTR O scale 2 rail equipment from a few major players still. Atlas, 3rd Rail, etc.
A few left us. MTH, Weaver, OMI, etc.
