Is the fan next to the smoke unit suppose to run while engine is running? It does smokes but not like I would want it to. When blow on it and it turns freely but does not run on its own.

Smoke on a subway? Maybe an actual MTH model number would be appropriate here. The only R-11 that the MTH site shows are subways.
The MTH model number is 20-20094-1. High Hood Diesel Engine #305 PS 2.
Oh, an RS-11, not an R-11, makes a difference. 
That one has the full sized smoke unit, so something is amiss, it should smoke you out of the room! If the fan isn't running, it'll have to be fixed. Could be something simple, but hard to know.
