No more than 35 miles, 27 of this is the Buzzards Bay to Hyannis "Cape Main" that is used by the 3-season Cape Cod Central (capetrain.com) and the summer-only MBTA CapeFlyer (capeflyer.com). A subsidiary freight operator, Mass Coastal, runs the approx. 8-10 mile Falmouth branch handling the daily waste to energy unit trains to a mainland destination, all other freight traffic appears to be very light.
Almost 100 miles of track were operated by the New Haven up to the 1940s. Much of this is now rail-trails.
My last sighting of a train on the Cape was around 1976, a Penn Central GE with a gaggle of freight cars crossing the Buzzards Bay Bridge.

Back in 2008 I bike most of the rail trail.  Nice the right of way is preserved but sad the rails are no longer there.  Did the dinner train, not bad.
